Health Share Plans
If you have a health share plans such as Medi-Share, Samaritan Ministries, Liberty Health Share or others, you will be required to pay for services rendered on the date of the service. The fees will be according to our cash pay pricing. You may then submit your claims to your Health Share for reimbursement.
Payment Policy
All copays are expected at the time of service. this is part of our contract with our insurance company. It is a violation of our contract to waive these fees or collect them at a later time. Please come to the office with your copay for each and every visit. Follow-up visits require copays as well.
Office Visits
You are responsible for all balances not paid by your insurance company.
Copays are collected after your office visit. We accept cash, checks, Visa, American Express, Mastercard, Discover, Debit cards, flex spending cards (with MC or Visa logo). Please bring your copay to each and every visit. This must be paid at the office after your visit, per your contract and our contact with the insurance company. Arrange settlement of your account within 30 days of your bill.
